    As the Hirer, you confirm that you are an authorised signatory and that the Agency Worker has completed the scheduled assignment(s).
    This timesheet serves as a condensed version of the Terms of Business of Chint Limited T/A Entire Care (Company No. 08920686),
    which are compliant with the Agency Worker Regulations (AWR). The full Terms of Business were provided prior to the commencement of services.

    The Hirer acknowledges that if they directly engage the Agency Worker introduced by the Employment Business,
    or introduce the Agency Worker to a third party resulting in engagement, a Transfer Fee will apply:
    15% of the Agency Worker’s remuneration during the first 12 months of engagement,
    or a fixed amount of £2,500 for Healthcare Assistants/Support Workers, and £4,500 for Qualified Nurses.

    The Engagement may commence by written agreement with the Employment Business for a minimum of 12 weeks at 36 hours per week.
    The Transfer Fee applies if the engagement occurs during the assignment, within the relevant period, or within six (6) months of introduction.
    By signing above, the Hirer confirms acceptance of these terms.
